People over the age of 50 make up 28% of the Chigwell population.  As people get older they sometimes need support to stay in their own homes.  Older people in our community have a wealth of lived experience and are a welcome asset to our community house.

We operate several activities to assist older people and people with a temporary disability to stay in their own homes.  These activities are funded under the CHSP and HACC funding streams.

WHAT IS THE CHIGWELL SUPPORT PROGRAM?

The Chigwell Support Program is a Commonwealth Home Support Program (CHSP) and Home & Community Care (HACC) funded service.

The Service endeavours to maintain the dignity of clients and enable them to remain in their own homes and communities.

We are based at the Bucaan Community House.

WHO IS ELIGIBLE FOR OUR SERVICE?

If you live in the areas of Montrose, Rosetta, Berriedale, Chigwell, Claremont, Austins Ferry or Collinsvale you may be eligible.

You also need to meet criteria for CHSP & HACC.

CHSP & HACC services are designed for people who need support to continue living in the community and who are frail, or have a disability.

CHSP & HACC services are also available for those who are caring for a frail older person or a person with a disability.

HOW DO I RECEIVE YOUR SERVICES?

Clients are now referred to us through external assessment organisations myagedcare or The Tasmanian Community Care Referral Service.

You will be assessed under the CHSP & HACC guidelines.
